Air Force & Army - PCS Orders with Names of Dependents
Marines - PCS Orders & Area Clearance
Navy - PCS Orders with Dependent Entry Approval
DOD Civilian - PCS Orders & Verification of Employment
Immunization Form (DSPA122.1): This form must be completed by the base medical clinic or the U.S. Naval Hospital on Camp Lester. Take your child and their immunization record (an appointment is not required). Bring the immunization record back for the school to make a copy.
Passport (Birth Certificate for Kindergarten students)
Student's Social Security Number
Address of previous school to request records.
Verification of Quarters Address: On-Base Housing Assignment Letter or Off-Base Lease or Utility Bill

IMPORTANT: Your child will attend the school that is ZONED for your TEMPORARY QUARTERS. When you have obtained off-base or on-base housing, your child may be zoned for a different school. If you are not certain what zone your house may be located in, contact the school office (634-1329), the District Superintendent’s Office (634-1204), or the School Bus Office (645-7820).
